DOT DER TRAINING COURSE

At 5 Star Workforce, all of our digital courses are delivered electronically and are accessible by invitation upon purchase. By enrolling in our online training programs, you agree to the following:

• You must provide the full first and last name of the individual who will be taking the course, along with a valid and active email address. This information is required to create your student profile, track progress and to generate a certificate of completion.

• Access to the course materials is granted via a secure digital portal, which may include video lessons, PDFs, quizzes, and certificates by email invitation. 

• Courses are intended for individual use only and may not be shared, copied, or redistributed.

• Completion certificates will be issued only after all required modules and assessments are completed.

• Course access is non-transferable unless explicitly approved in writing.

• Some courses may be subject to expiration dates or access limits as stated in the course description.

• Certificates will be issued only to the named and registered participant upon successful completion of all required modules.

• Must purchase two course minimal

We are committed to providing quality, accurate, and up-to-date training in accordance with regulatory and industry standards.

• Successful Designated Employer Representative (DER) students will be able to manage a drug and alcohol free workplace policy within their organization including providing the necessary training to employees and making determinations of when to test and when a refusal should be determined.

• Successful students will be able to express an understanding of industry terms and the protocols involved with the DOT and PHMSA regulations by completing an online quiz with 75% or greater proficiency.

Due to the instant and irrevocable access provided upon purchase, all digital course sales are final. No refunds, exchanges, or cancellations will be granted once a course has been purchased.

• You understand that providing accurate participant information (first and last name + email address) is required to receive access.

• You agree that all course content is for personal, non-commercial use, and may not be shared or redistributed.

Please Read Before Purchasing:

Once a digital course is purchased from 5 Star Workforce, you will receive a Course Enrollment Questionnaire via email. This form is required to collect the necessary information to create your student portal access on our training platform.  ➡️ Course access will not be granted until the completed questionnaire is submitted.Information you will be asked to provide includes:

• Full legal first and last name of the student

• Valid email address (used for login and certificate delivery)

• Course purchased

• Additional participant details (if applicable)

This step ensures secure access, accurate tracking, and proper certification for each participant. If you do not receive the questionnaire within 15 minutes of your purchase, please check your spam/junk folder or contact our team at support@5starworkforce.com

🔹 Policy Details

• Two-Course Minimum Requirement Certain training programs are sold in 2-course minimum bundles, whether for DOT compliance, supervisor onboarding, or workforce team training.

• Use One Now, One Later If you choose to enroll one student immediately and hold the second course for later, that’s perfectly fine! Your second course remains available for future use at no additional cost.

• No Expiration Your unused course credit does not expire. You may redeem it at any time when ready to enroll your second participant.

How to Redeem the Second Course When you're ready to enroll the second student, please email the following information to support@5starworkforce.com: 

• Subject line: Second Course Enrollment Request

• Company Name

• Manager/Purchaser Name

• Course Title

• Student Full Name (First & Last)

• Student Email Address (for course login)

• Once received, our team will create the student portal and email login credentials within 24 business hours.

• Non-Transferable Second-course access may only be used by employees within the same company that made the original purchase. Courses are not transferable between companies.
